How It Works
A Personalized 12-Month Program for Real, Lasting Change
Your health is not fixed in one rushed appointment.
Habits don’t change overnight.
Adding years to your life requires daily thought change, vigilance, dedication, and frequent, expert testing.
Those things are big, and can be scary, and they DON’T happen alone.
They require a partner, support, willingness to make changes, re-examine things, be thoughtful and honest with yourself.
Whether you are navigating weight changes, perimenopause,
menopause, or overlapping concerns, we work with you over
the course of one full year to create a plan that is
thoughtful, medically sound, and built for real life. Our direct-
care model is designed to give you the time, guidance, and on-
going support needed to make meaningful progress.
The Equilibrium Health Method
This structure is designed to make the process feel easy to understand while showing the depth of support patients receive during the year.
-
Patients begin by completing a brief application or booking a consultation online.
This first step helps clarify goals, symptoms, readiness, and fit. It should feel easy, welcoming, and low-friction.
What this includes:
Simple online application or booking flow
Opportunity to share primary concerns and goals
Clear explanation of next steps and program fit
-
Care begins with a deep-dive physician intake visit. This is not a rushed primary care appointment. It is a thorough conversation about symptoms, medical history, prior treatment, lifestyle patterns, labs, stressors, and what is and is not working in the patient's daily life.
What this includes:
Weight history, appetite, metabolism, and prior strategies
Perimenopause or menopause symptoms, cycle changes, sleep, mood, cognition, libido, and energy
Lifestyle, stress, movement, nutrition, and daily barriers
Goals, priorities, and concerns that need a tailored pla
-
After the intake, the patient receives a tailored medical plan and, when appropriate, lab recommendations. This is where information becomes action.
The plan should feel specific, realistic, and adaptable.
What this includes:
Treatment strategy personalized to symptoms and goals
Recommended labs and interpretation when appropriate
Education on the why behind the plan
Clear roadmap for the next several months
-
This is where real progress happens. The program is intentionally built as a year- long relationship so care can be adjusted over time. Patients continue to receive physician follow-up, topic-specific coaching, unlimited messaging, educational support, and troubleshooting as life changes.
What this includes:
Follow-up physician visits every 6-8 weeks on average
30-60 minute follow-up visits with flexibility based on clinical need
Physician-coach-led topic-specific coaching that can be as often as weekly when needed
Unlimited text access between visits
Ongoing review of symptoms, adherence, obstacles, labs, and treatment response
-
As the year progresses, the focus expands beyond short-term symptom relief to sustainable health. The care plan continues to be refined around what is happening in the patient's real life so that treatment remains relevant, effective, and personalized.
What this includes:
Medication and hormone plan optimization
Lab review and treatment adjustments
Longer-term weight, symptom, and lifestyle strategies
A maintenance plan for care after the first year
